Home and living assessments

For people with disabilities, at home across greater Brisbane or by telehealth anywhere in Australia.

When someone needs more support at home, or a different living arrangement, the NDIA needs clear evidence of what support is needed, when and why.

Our Home and Living assessments provide that evidence, built around the person’s goals and what a typical day and night actually look like for them. We work with adults who are seeking Supported Independent Living (SIL) or an Individualised Living Option (ILO), moving out of the family home or whose informal supports are changing, having a Home and Living request reviewed after a decline, or needing their current SIL hours reviewed.

Clear evidence of the support needed, day and night.

What to expect

  • A phone call with you or your support coordinator to understand what the report needs to show and any review dates

  • With your consent, we read existing reports and speak with people who know you well, such as family, support workers and your treating team

  • The assessment, at home or somewhere you feel comfortable. This is usually one two-hour visit, or two shorter visits if that suits you better, with breaks whenever you need them. You’re welcome to have a support person with you

  • Your report, with a quick turnaround and clear recommendations linked to your goals

We usually take on new assessments within about four weeks, and can commit to a set completion date when a plan review is coming up. Home and Living assessments are funded through NDIS Capacity Building (Improved Daily Living) for self-managed and plan-managed participants.
